Welcome to the Class D - Adult League Survey!

Please complete this survey with the information you can. Only 1 survey per team, please. Information, such as costs, will be sent out to all respondents as well as posted to the OHSBL website as well as this one. The team fee will be $500 per team and will include insurance and a double-elimination State Championship Tournament.
